The United States EMS Compact gives your state a modern tool for a flexible, mobile, and accountable EMS workforce. It is enacted as state law and functions as a binding agreement among participating states, authorized under the Compact Clause of the U.S. Constitution (Article I, Section 10, Clause 3). The Compact has been enacted in 28 states and is operational in 27, and is administered by the Interstate Commission for EMS Personnel Practice, a governmental body on which each participating state holds one vote.

What the Compact Does for Your State
- A reliable workforce. Your state gains access to a qualified workforce of more than 480,000 EMS clinicians who meet common standards for education, testing, and background checks, supporting rural coverage, vacancy backfill, and large events.
- Stronger public protection. Your State EMS Office gains secure access to the National EMS Coordinated Database, with authority to track and share licensure history, receive timely notice of disciplinary actions, conduct cross-border investigations, and issue subpoenas across state lines.
- Readiness without delay. The Compact enables immediate mutual aid during disasters, mass gatherings, or major incidents, without waiting for emergency declarations or temporary waivers.
- A voice in national EMS policy. Each participating state appoints a voting Commissioner who helps shape national solutions while protecting local interests.
- No fees. There are no fees to states or to EMS clinicians.
How Your State Joins
A state joins by enacting the REPLICA model legislation in its uniform form, appointing a Commissioner, and integrating with the National EMS Coordinated Database.
